By signing this permission slip, I am committing to the following guidelines:

Behavior Expectations on the Overnight Trip

Each student must comply with all applicable school behavior guidelines and rules while participating on the overnight trip. School staff may separate any student failing to do so and may contact the parent/guardian as soon as feasible. Upon request by a school staff member, the parent/guardian must arrange to have the student picked up as directed by the staff member. If a pick up cannot be feasibly arranged, school staff may arrange for alternative transportation at the expense of the parent/guardian. Depending on the severity of the behavior, the student may also be subject to additional and subsequent disciplinary measures.

Student Eligibility for Overnight Trips

1. Student must be in good academic standing (grades 5-12).

a. Student may not be failing any classes (F1 grade – cumulative grade for the year) - grades will be checked two weeks prior to field trips/overnight trips. If student is failing any classes at that time they may not be able to attend the trip or school event.

2. Students must meet the following behavior requirements:

a. Student may not have accrued more than 10 After School Detentions during the school year for any reason

b. Student may not have more than 6 Full Day In-School Suspensions

c. Student may not have 3 or more out of school suspensions

d. Students may not have a 10-day suspension

3. Student must meet the following attendance requirements:

a. Students may not have more than 15 absences (excused and unexcused)

b. Students may not have more than 20 unexcused tardies or early dismissals

4. Permission slip must be received by the due date

Payment must be paid on time per due dates scheduled in field trip and overnight trip information
Students may not have any outstanding fees due – including but not limited to technology, repairs, club dues, athletic fees, etc.

All final decisions regarding student participation on overnight trips are at the discretion of the administrative team.

Parental Consent for Treatment if Needed

In the event your child becomes ill or injured while attending the Bradford Preparatory School fall retreat. I give permission for a Bradford Prep staff member to take my child to the nearest medical facility or healthcare provider and have the necessary treatment administered. My signature verifies that my child is in good health and capable of participating in strenuous activities. My signature also acknowledges my acceptance and understanding of Bradford Preparatory School’s role in the medical treatment of your child. In case of emergency, I understand that every effort will be made to contact me. If I cannot be reached, I hereby give Bradford Preparatory School the permission to act on my behalf in seeking emergency treatment for my child in the event that such treatment is deemed necessary by Bradford Preparatory School. I hereby give permission to the medical personnel selected by Bradford Preparatory School to order x-rays, routine tests, treatment; maintain and/or releases any medical records necessary for insurance purposes as outlined under the HIPAA regulations. I absolve Bradford Preparatory School from liability in acting on my behalf in this regard. I hereby release and hold harmless Bradford Preparatory School, its officers, agents, and employees from any liability or damages, and I hereby waive all claims or causes of action against Bradford Preparatory School, its officers, agents, or independent contractors, which may result from participating in the school-sponsored trip/camp and/or the administration of medication as described above.

The school shall not be responsible or liable for any injury/accident via commercial transportation. This includes transportation via any non-school owned, leased, or operated vehicle and includes drivers who may not be employed or under control of the school.

Medications Needed on the Trip

Students requiring prescription OR over-the-counter medications during the trip must have a current health assessment and medication administration form for each medication on file in order to bring medications on the overnight trip.

COVID Protocols on the Trip

The Board of Directors approved the following guidance regarding COVID for the 2021-2022 school year. COVID Protocols will be followed on all field trips and overnight trips:

All staff, students and visitors must wear a mask indoors during school hours. Masks are not required outdoors.

· Quarantine Protocols will be followed for an individual who has been in close contact (within 6 feet for at least 15 minutes cumulatively over a 24-hour period) of someone who is determined positive with COVID-19 either through testing or symptom consistent diagnosis, with the following three exceptions:

· Individuals who are fully vaccinated and do not have symptoms do NOT need to quarantine after a close contact.

· People who have tested positive for COVID-19 within the past 3 months and recovered and do not have symptoms do NOT have to quarantine.

· Students who are not fully vaccinated after a close contact in a classroom or other school setting if masks were being worn appropriately and consistently by both the person with COVID-19 and the potentially exposed person do NOT need to quarantine.

CDC continues to recommend quarantine for 14 days after last exposure. However, as of December 2, 2020, the CDC has offered options to reduce the duration of quarantine in either of the following two scenarios:

10 days of quarantine have been completed and no symptoms have been reported during daily at home monitoring;
7 days of quarantine have been completed, no symptoms have been reported during daily at home monitoring, and the individual has received results of a negative antigen or PCR/molecular test on a test taken no earlier than day 5 of quarantine.
If quarantine is discontinued before day 14, the individual should continue to monitor symptoms and strictly adhere to all non- pharmaceutical interventions (e.g. wear a mask, practice physical distancing) through 14 days after the date of last exposure.
